This report presents the findings of a presidential commission created to examine US intelligence failures in the run-up to the invasion of Iraq. The report also investigates the performance of US intelligence agencies in Iran, North Korea, and Libya, and examines the WMD capabilities of the al-Qaida organization. The authors offer recommendations for improving intelligence gathering and analysis, and conclude that dramatic changes are needed to prevent failures similar to the fiasco over Iraq's missing weapons.
