This paper sets out a framework for the debate on how best to develop health and social security systems by examining the case for and against universalist systems that provide comprehensive services as well as systems that provide targeted or selective services. The author also examines the different assumptions that underlie these two systems, the policy implications of adopting them, and the issues that should be considered by policy-makers in emerging economies who are seeking to expand their health and social security coverage.
